Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The
employer did not furnish employment and a place of employment which were
free from recognized hazards that were causing or likely to cause death
or serious physical harm to employees in that employees were exposed to:
a) 2/29/88 - Employee(s) were exposed to: The hazard of the valve(s) of
100lb. LPG cylinders being physically damaged thus causing an
uncontrollable release of propane and creating a potential for a fire
and or explosion, which could cause serious physical harm or death.
Among other methods to correct this hazard is protecting the valve(s) -
(1) By setting the valve(s) into the recess of the container to prevent
the possibility of their being struck if the container is dropped
upon a flat surface;
(2) By ventilated cap or collar, fastened to the container capable of
withstanding a blow from any direction equivalent to that of a 30
lb. weight dropped 4'; construction must be such that the blow will
be transmitted to the valve(s). Location: Jobsite filling station.
