Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not furnish employment and a place of employment which
were free from recognized hazards that were causing or likely to cause
death or serious physical harm to employees in that employees were
exposed to:
....the hazard of falling from bucket(s) of front end loaders.
Employee(s) obtaining access to elevated work platforms throughout
the facility by being lifted inside a front end loader bucket, model
#988-B, SN 50W2958, were exposed to the hazard of an 18 foot to 20
foot fall from elevation resulting in crushing injuries, broken bones,
severe trauma and/or death.
One feasible and acceptable abatement method, among others, to correct
this hazard is to provide alternative means of access to elevated
work areas by using an approved articulating boom bucket truck.
