Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not furnish employment and a place of employment which
were free from recognized hazards that were causing or likely to cause
death or serious physical harm to employees in that employees were
exposed to:
....from the hazard of being struck by falling portions of the
crane structure and crane load during lifting operations with the
twenty (20) ton capacity electric overhead traveling bridge crane
number 73, located in the Hot Roll Finishing Area of the old 3/8 inch
Line Building, due to an unanticipated failure of a portion of the
crane or of the hoist line due to an insufficient examination of the
equipment prior to hoisting roll buggies weighing in excess of the
crane's rated capacity.
Among other means, one feasible and acceptable abatement method to
correct this hazard is by conducting a comprehensive survey of the
condition of all overhead traveling cranes in accordance with the
provisions of Subsection 2-3.2.1.1 Special Heavy Lifts of American
National Standard B30.2.0-1976 Overhead and Gantry Cranes (Top Running
Bridge, Multiple Girder), including (but not limited to):
1. A comprehensive review of the maintenance history of the crane
performing the lift (paragraph 2-3.2.1.1(a));
2. An inspection of the structural, mechanical and electrical
components of the crane's design according to accepted crane design
standards such as CMAA 70 or AISE 6, performed by the crane's
manufacturer or other qualified persons (paragraph 2-3.2.1.1(b));
3. A survey of the crane supporting structure for conformance to AISC
or other applicable design standards (paragraph 2-3.2.1.1(c));
4. A complete inspection of the crane as specified in Subsection
2-2.1.3 of ANSI B30.2.0-1976, made immediately prior to the lift
(paragraph 2-3.2.1.1(d));
5. Maintenance of complete records of the lift, including all
associated inspections, on file where they are readily available
(paragraph 2-3.2.1.1(g));
6. A thorough inspection of all critical parts of the crane after
the special heavy lift is concluded (paragraph 2-3.2.1.1(h)).
