Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to: (a) Injuries from falling through the upper level floor opening for the south lowerator in the MPF Building to the lower level floor approximately 22 feet below, through the unguarded east and west entrance openings. Among other methods, one feasible and acceptable method to correct this hazard is: (1) To provide gates which are equivalent to standard railings and would be in place across entrance openings, at times when the openings were not being used by the lowerator.

Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to: (a) Crushing injuries from working under a load being hoisted by an underslung crane, Unit #K333, at the main production plant, Plant #2. Among other methods, one feasible and acceptable abatement method to correct this hazard is: (1) To ensure that loads are not hoisted over employees and that employees do not step under hoisted loads.
