Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The

employer did not furnish employment and a place of employment which
were free from recognized hazards that were causing or likely to cause
death or serious physical harm to employees in that employees were
exposed to:
(a)  Jobsite at Walnut and 11th Street, Kansas City, Missouri.
On August 23, 1991, at 9:55 a.m., an employee was observed being
lowered from a 30 foot level to ground level in a crane suspended
platform, which also contained a crate of unknown weight.  The
crane and rigging used to hoist the platform were deficient as
follows:
(1)  The hook was not provided with a positive locking type
latch - it was equipped with a spring loaded type latch only,
exposing employees to the possible hazard of a dropped plat-
form were the wire rope to lose tension momentarily due to
occurrence such as malfunction or catching the platform on a
building ledge;
(2)  The wedge and socket attachment of the overhead ball was
not equipped with a wire rope clip on the free end of the
load line;
(3)  No anti-two blocking device was provided on the crane
boom, exposing employees to the possible hazard of a dropped
platform due to raising the platform to a height where the
load block would interfere with the boom tip.
Among other methods, one feasible and acceptable abatement method to
correct this hazard is to install the correct type of safety latch and
to install an anti-two blocking device, in accordance with ASME/ANSI
B30.5-1989, Section 5-3.2.2, "Safety Standard for Cableways, Cranes,
Derricks, Hoists, Hooks, Jacks and Slings - Mobile and Locomotive
Cranes".
(a)  Essential elements include:
(1)  Install a positive locking safety latch on wire ropes
used to hoist personnel platforms and install an anit-two
blocking device to prevent two block damage to wire rope or
rigging attachments.
Another possible abatement method would be to ensure that cranes are
not used in personnel hoisting operations unless in compliance with
all applicable safety standards and regulations.
