Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to in confined spaces with potential for atmospheric hazard, the permit space atmosphere shall be tested prior to each entry in the following sequence: Oxygen concentration, combustible gas or vapor, and potential toxic contaminants: (a) On 06/24/92, an I & C employee entered a gas vault without checking for oxygen concentration.
