Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not furnish employment and a place of employment which
were free from recognized hazards that were causing or likely to cause
death or serious physical harm to employees in that employees were
exposed to:
(a)  On 4/27/92 and 5/6/92, employees were exposed to the hazard
of serious injuries from falling from either the forks of
fork lifts or pallets used as elevating platform on fork
lifts, while employees were riding on them picking orders
and unjamming conveyors. Secured safety platforms were not
utilized on the forks of Clark fork lifts in the warehouse.
Feasible and acceptable methods among others to correct
ths hazard is to:
1) Utilize a safety platform secured to the lifting carriage
and/or forks which meets the requirements of ANSI
B56.1-1975 Part II - 427A and 427C.
2) Maintaining an operator in the control position on the
truck.
3) Provide overhead protection as necessary as operating
conditions indicate, if used for order picking.
4) Instruct and train employees in the proper use of fork
lifts including use of safety platforms.
5) Maintain any reference material as necessary, such as
operating manuals, for employee use.
