Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employmnent and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to fire and explosion hazards:
a) Production Area - Reactor #2 - Pressure vessel, being used as a
reactor, was not
provided with a pressure relief device to prevent over pressurization and
vessel failure.
On or about 06/16/95.
Among other methods, one feasible and acceptable abatement method to
correct this hazard is:
1) Follow the requirements of the American Society of Mechanical Engineers
(ASME)
Boiler and Pressure Vessel Code, Section VIII, Division I, 1992.
2) Provide pressure vessels with pressure relief devices (protective
devices), properly
designed and installed prior to initial operation, that meet the
requirements of Sections
UG-125 through UG-136 of the above code.
3) The relief devices shall be vented to a safe location, preferably to
the outside. Such
devices shall be inspected and maintained in accordance with the
manufacturers
recommendations and/or as needed based on what is dictated by the process.
