Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not furnish employment and a place of employment which were
free from recognized hazards that were causing or likely to cause death or
serious physical harm to employees in that employees were exposed to th
hazard of falling from the basket of a "Versalift" Aerial Lift Truck
#3016506,
which had a damaged and defective access door.
a)  On or about 9/12/92, At Beach 17th Street & Brookhaven Avenue

approximately 15 feet above the ground.
Among other methods, one feasible and acceptable abatement method to
correct
the hazard would be to remove the lift truck from service until the new
door, which had been ordered, was received and installed.  In the
alternative, employees should have been advised that the access door was
defective and should have been given specific instructions to utilize the
other personal protective equipment provided (i.e. safety belt, lanyard,
safety chain).
