FREE FROM RECOGNIZED HAZARDS THAT WERE CAUSING OR LIKELY TO CAUSE DEATH
OR SERIOUS PHYSICAL HARM TO EMPLOYEE IN THAT EMPLOYEE WERE EXPOSED TO
FALL HAZARDS FROM CATENARY SCAFFOLDS WHICH DID NOT CONFORM TO ANSI
A10.8-1977;
A) WESTSIDE OF ROOSEVELT AVENUE - ON 8/27/90 -
EMPLOYEES WERE WORKING FROM 5 CATENARY SCAFFOLDS WHICH WERE ALL
APPROXIMATELY 20 FEET FROM STREET LEVEL. THEY WERE PAINTING THE
ELEVATED TRAIN TRACKS. EACH CATENARY SCAFFOLD RESTED FREELY ON
5/8TH 10 TON WIRE ROPE. THE SCAFFOLDS WERE NOT PROVIDED HOOK SHAPE
STOPS ON EACH END SO AS TO PREVENT THEM FROM SLIPPING OFF THE WIRE
ROPE. VERTICAL PICKUPS WERE NOT PROVIDED EVERY 50 FEET TO PICK UP
THE SAG OF THE WIRE ROPE. IN ADDITION, SAFETY BELT LANYARDS WERE
NOT ATTACHED TO AN INDEPENDENT LINE. THE LANYARDS ARE ATTACHED TO
THE WIRE ROPE WHICH IS SUPPORTING THE SCAFFOLD. THE EMPLOYEES WERE
EXPOSED TO THE HAZARD OF FALLING UP TO APPROXIMATELY 20 FEET TO
STREET LEVEL.
ONE FEASIBLE AND ACCEPTABLE METHOD TO CORRECT THE HAZARD IS TO FURNISH
AND UTILIZE THE HOOK SHAPE STOPS, VERTICAL PICKUPS AND AN INDPENDENT
LINE FOR THE ATTACHMENT OF BELT LANYARDS. REFERENCE: ANSI A10.8 1977
