Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The
employer did not furnish employment and a place of employment which were
free from recognized hazards that were causing or likely to cause death
or serious physical harm to employees in that employees were exposed to:
a) 1/10/90, Employees were exposed to serious injuries form an
inadvertent fall of 10 foot into an open elevator shaft, due to
lack of interlocks with electrical contacts.
One feasible and acceptable abatement method to correct this hazard is
to provide interlocking doors or gates at the elevator entrances as
stipulated by ANSI-A 17.1, Section 110.1(E).
Second floor storage room - Two doors at the elevator entrance were
opened.
