Section 5(a)(1) of the Occupational Safty and Health Act of 1970:  The
employer did not furnish employment and a place of employment which were
free from recognized hazards that were causing or likely to cause death
or serious physical harm to employees in that employees were exposed to:
On or about May 19, 1988
a) Employees were exposed to a type II 'D' Fault: Fire, possible
explosion from 27 Kilovolt-17.5 Kiloamp on an energized feeder.
The feeder cable 5Q31 located in manhole #56190 at the north side
of Linden Blvd., 40 ft. east of 159th St., Jamaica, N.Y. 11434, was
not immediately removed from service and repaired as required in
Con Edison EO-1184-4 (1/29/88), "Distribution Underground Cables
and Joint Inspections 'C' and 'D' Faults." Employees were engaged
in blanketing the swollen joint in order to work on a burned out
feeder cable in same manhole.
Among other methods, one feasible and acceptable abatement method to
correct this hazard is to remove 'D' Fault from service before
entering manhole;  Do not enter manhole without de-energizing the
feeder cable.
