Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The
employer did not furnish employment and a place of employment which were
free from recognized hazards that were causing or likely to cause death
or serious physical harm to employees in that employees were exposed to:
On 8-12-88
Location: Bruckner Blvd. and Alexander Ave.
a) Employees were exposed to a type 11 "D" fault line, possible
explosion from 13,000 volts on an energized feeder cable. The feeder
cable 3M41 located in Manhole #509 was not immediately removed from
service and repaired as required in Con Edison EO-1184-3 (1/29/88).
Operation and Maintenance of Cable Procedures, Distribution and
Underground Cable Joint Inspections "C" and "D" Faults. Employee
was engaged in blanketing the swollen joint in for other employees
to work in manhole.
Among other methods, one feasible and acceptable abatement method to
correct this hazard is:
Remove "D" Fault from service, do not enter manhole without
Denergizing the feeder cable.
