Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not furnish employment and a place of employment which were
free from recognized hazards that were causing or likely to cause death
or serious physical harm to employees in that employees were exposed to
a fall hazard from elevated unprotected forks of a forklift.
On 11-14-88
a) Forklift truck operator observed standing on the fork tines of an
elevated powered industrial truck. Employee exposed to falling
approximately 10 feet while moving a shipping container from the
highest level of a rack in storage area for shipping on the closed-
up fork tines of a Clark Fork Lift Truck Model C500-25 S/N 235-1062-
2321.
Among other methods, one feasible and acceptable abatement method to
correct this hazard is to use a safety platform secured to the lifting
carriage and/or forks as stipulated by ANSI B 56.1-1975, section 427
and 513 A, American National Standard for powered industrial trucks (low
lift and high lift trucks).
