Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the hazard of falling: a) On or about 6/20/03 at the Dollar General Store, 1320 Washington Street, Jamestown, NY., employees were exposed to fall hazards of eighteen (18) feet, nine (9) inches to the ground below while climbing up and down on the guardrail system of the JLG Scissors Lift to access the roof trusses. AMONG OTHER METHODS, ONE FEASIBLE AND ACCEPTABLE ABATEMENT METHOD TO CORRECT THIS HAZARD IS FOR THE EMPLOYER TO CONDUCT TRAINING ON THE SAFE USE OF THE SCISSORS LIFT AS SPECIFIED BY ANSI A92.6-1999, SECTION 8.5. NO ABATEMENT CERTIFICATION REQUIRED

Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the hazard of being crushed/struck by falling trusses: a) On or about 6/18/03 at the Dollar General Store, 1320 Washington Street, Jamestown, NY., proper bracing of trusses was not performed causing them to collapse. AMONG OTHER METHODS, ONE FEASIBLE AND ACCEPTABLE ABATEMENT METHOD TO CORRECT THIS HAZARD IS TO: 1) UTILIZE THE RECOMMENDATIONS OF THE TRUSS PLATE INSTITUTE WHICH WERE CONTAINED IN HIB-91 BOOKLET, RECOMMENDATIONS FOR HANDLING, INSTALLING, & BRACING METAL PLATE CONNECTED WOOD TRUSSES DELIVERED TO SITE WITH THE TRUSSES. 2) FOLLOW THE SHOP DRAWINGS PROVIDED BY THE MANUFACTURER OF THE TRUSSES. 3) FOLLOW RECOMMENDATIONS SET FORTH BY DESIGNING ENGINEERING FIRM. NO ABATEMENT CERTIFICATION REQUIREDATION
