Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to: a) On or about 8/18/03 behind 90 Foxboro Lane, Amherst, NY., fiberglass extension ladder was not footed by another employee or tied-off causing it to slid down the CATV line and fall to the ground causing the employee to sustain serious injuries. AMONG OTHER METHODS, ONE FEASIBLE AND ACCEPTABLE ABATEMENT METHOD TO CORRECT THIS HAZARD IS TO: 1) PROHIBIT EMPLOYEES FROM CLIMBING LADDERS UNLESS TIED-OFF OR FOOTED BY ANOTHER EMPLOYEE. 2) TO TRAIN OR RETRAIN EMPLOYEES IN THE PROPER USE OF LADDERS. 3) MAKE CERTAIN THE BASE OF THE LADDER IS IN CONFORMANCE WITH ANSI-A-14.5-2000, SECTION 9.3.4 SAFETY REQUIREMENTS FOR PORTABLE REINFORCED PLASTIC LADDERS. NO ABATEMENT CERTIFICATION REQUIRED
