Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to the hazard of being struck or crushed by falling auto lift:
a)842 11th Ave., New York, N.Y. - The employer failed to ensure that the
superstructure bolts on the "Globe" model FS-10 series hydraulic
automotive lift
were tight and torqued at 125 foot pounds. The employer also failed to
perform
periodic inspections and establish and implement  a preventative
maintenance
program for the lift;  on or about July 12, 1995.
ABATEMENT NOTE: Among other methods,one feasible and acceptable means of
abatement
would be to establish and implement a preventative maintenance program
which would include
periodic inspection and checking the tightness of superstructure bolts for
proper
torque as
required by the manufacturer and referenced in section 3.1 of the Globe
Installation, Operation
and Service Part Manual- Model FS-10 Series,and referenced in American
National Standard
for Automotive Lifts-Safety Requirements for the Construction, Care, and
Use -  ANSI/ALI
B153.1-1990, section 7.3 and Standards for Automotive Lifts - Safety
Requirements for
Operation, Inspection, and Maintenance (ALI ALOIM-1994) - sections 5.1,
5.6.2.6, and 6.1.1
as published by the American Lift Institute.
