Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employmnent and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to: The hazards of hands or freight being caught between decending or ascending elevator car and shaft wall due to the fact that the elevator car wall perforations (openings) were greater than 1.5 inches wide. a)Elevator at 1155 Manhattan Ave. On or about 9-8-95 through 10-19-95. Among other methods, one feasible and acceptable means of abatement would be to make elevator car perforations less that 1.5 inches wide. This feasible and acceptable means of abatement is listed in the following reference: Reference; ASME A.17.3 - 1973 3.4.1.

Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employmnent and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to: The hazard of the elevator car falling as a result of being overloaded due to the fact that the elevators did not have standard load capacity plates and standard "No Passenger Signs". a)Elevator at 1205 Manhattan Ave. and 1155 Manhattan Ave., on or about 9-8-95 through 10-19-95. Among other methods, one feasible and acceptable means of abatement would be to install standard load capacity plates and "No Passenger" signs inside each elevator. This feasible and acceptable means of abatement is listed in the following reference: Reference: ASME A.17.3 - 1973 3.7.4-5.e:
