Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The
employer did not furnish employment and a place of employment which were
free from recognized hazards that were causing or likely to cause death
or serious physical harm to employees in that:
a) Employees working in Department 11038 on the No. 982 B.P. EGR valve
cover and housing spin and assembly fixture on 9/12/89, were exposed
to the hazard of being caught between the spin head and the tooling
plate bushing due to the accidental or inadvertent activation of the
spin head while the employees were adjusting the slide with air
pressure supply to the machine.
Employees could suffer severe crushing and/or amputations to fingers
and hands.
Among other methods, one feasible and acceptable method to correct this
hazard is to:
1) Enforce the plant lockout program;
2) Provide for periodic inspections to verify compliance with the plant
lockout/tagout program as stipulated by American National Standard
Institute Z244.1 1982 for personnel protection, lockout/tagout of
energy sources, minimum safety requirements.
