Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The
employer did not furnish employment and a place of employment which were
free from recognized hazards that were causing or likely to cause death
or serious physical harm to employees in that:
a) Employees working in Building #6, Dept. 674 were exposed to live
electrical parts of panel control unit. Employees were required on
8/1/89 to install an air conditioning unit, wiring and transformer,
for a piece of engineering test equipment in existing power unit.
Among other methods, one feasible and acceptable method to correct this
hazard is to: Enforce an adequate plant lockout procedure such as that
stipulated by the GM-UAW National Joint Health and Safety Committee,
requirements for lockout/tagout of electrical elements of the lockout
procedure would include but not be limited to:
1) Develop a flow chart diagram showing the steps that make up a
process.
2) Use proper symbols to designate a step or action to be taken.
3) Identify the energy source and equipment to be worked on.
4) Open and individually lock out the main power disconnect for the
equipment.
5) Test circuit to confirm its been disconnected, and as stipulated in
ANSI Z244.1 1982.
