death or serious physical harm to employees in that employees were
exposed to:

a Rigid 300 Electric Pipe Threading Machine, which was not provided
with a constant pressure foot treadle control switch as required by
American National Standards Institute B208.1-1982, Sec. 4.4, which
is one feasible and acceptable method of abatement.
