Section 5(a)(1) of the Occuaptional Safety and Health Act of 1970:  The
employer did not furnish employment and a place of employment which were
free from recognized hazards that were causing or likely to cause death
of serious physical harm to employees in that employees were exposed to;
crushing hazards.  Elevator cars were not provided with car gates or
doors at each entrance (American National Standards Institute/American
Society of Mechanical Engineers A17.3-1990 Section 3.4.2).
a)  At the freight elevators, employees who worked or rode
on the elevators were exposed to being caught between
the landing sills and the moving elevator cars, which
were not provided with car gates or doors at each
entrance.  On or about 6/3/93.
ABATEMENT NOTE:
Among others, a feasible method to eliminate or to
materially reduce this hazard is to comply with
ANSI/ASME A17.3-1990, Section 3.4.2 and provide car
gates or doors at each entrance to the car of the
freight elevators.
