Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The
employer did not furnish employment and a place of employment which were
free from recognized hazards that were causing of likely to cause death
or serious physical harm to employees in that employees were exposed to:
a) On or about 9/12/90, employees were exposed to the hazard of fire
and/or explosion while engaged in harvesting silicon carbide from
an electrical induction furnace. Complete plans of remodeling
existing equipment, sequence of operations and specifications were
not submitted for approval of the authority having jurisdiction.
Among other methods, one feasible and acceptable abatement method to
correct this hazard is to have plans drawn and show detail for approval
to the authority having jurisdiction, such as the stipulated by National
Fire Protection Association, NFPA, Number 86, 1985 Edition, and
subsequent updated NFPA 86, Section 1.5 Approval, Plans and
Specifications. essential elements for this procedure would include:
1) Have plans drawn and show all essential details as to locations,
construction, ventilation, piping and electrical safety equipment.
2) Provide wiring diagrams and sequence of operations for all safety
controls.
3) If deviations are made from the referenced standard, special
permission is required from authority having jurisdiction.
4) Ensure that all wiring in and around furnaces is in accordance with
National Electrical Code, NFPA 70 where it may be required to provide
greater safety of the installation under the code, and it shall meet
with the approval of all parties having jurisdiction.

Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The
employer did not furnish employment and a place of employment which were

or serious physical harm to emloyees in that employees were exposed to:
a) On or about 9/12/90, employees were exposed to the hazard of
fire and/or explosion while engaged in harvesting silicon carbide
from an electrical induction furnace. Employees were not thoroughly
trained as to the nature of the hazards and conditions involved in
this process.
Among other methods, one feasible and acceptable abatement method to
correct this hazard is to establish and enforce an adequate training
program such as that stipulated by National Fire Protection Association,
NFPA, Number 86, 1985 Edition and subsequent updated NFPA 86 Editions;
Section 1-6 Operator and Maintenance Personnel Training, essential
elements of the training program would include:
1) Thoroughly instruct and train all personnel under supervision of
experienced person(s).
2) Reqiure personnel to demonstrate an understanding of the equipment
and it's operation to assure knowledge of and practice of safe
operating procedures.
3) Establish regular retraining and testing to maintain a high level
of proficiency and effectiveness.
4) Provide employees access to operating instructions at all times.
5) Operator training shall include, where applicable, combustion of
fuel-air mixtures, expolosion hazards, sources of ignition, functions
of control and safety devices, handling of special atmospheres and
the handling and processing of hazardous materials.
6) The equipment manufacturer shall provide operating instructions.
These shall include: schematic piping and wiring diagrams, start-up
procedures, shut down procedures, emergency procedures including
those occasioned by loss of special atmospheres, electric power, or
other essential utilities and maintenance procedures.

A) ON OR ABOUT 9/21/90, EMPLOYEES WERE EXPOSED TO THE HAZARD OF FIRE
AND/OR EXPLOSION WHILE ENGAGED IN HARVESTING SILICON CARBIDE FROM
AN ELECTRICAL INDUCTION FURNACE. MONITORING WAS NOT PERFORMED FOR
CARBON MONOXIDE GAS THAT WAS CREATED BY A MIXTURE OF PULVERIZED COAL
AND SILICA. ELECTRICAL INDUCTION FURNACE OPERATING AT A TEMPERATURE
OF 2440 DEGREES FAHRENHEIT. CARBON MONOXIDE GAS WILL IGNITE AT A
TEMPERATURE OF 1128 DEGREES FAHRENHEIT.
AMONG OTHER METHODS, ONE FEASIBLE AND ACCEPTABLE ABATEMENT METHOD TO
CORRECT THIS HAZARD IS TO INSTALL SAFETY EQUIPMENT, SUCH AS THAT
STIPULATED BY NATIONAL FIRE PROTECTION ASSOCIATION, NFPA, NUMBER 86,
1985 EDITION, AND SUBSEQUENT UPDATE 86 EDITION, CHAPTER 4, SAFETY
EQUIPMENT AND APPLICATION WHICH STIPULATES:
1) FOR THE PROTECTION OF PERSONNEL AND PROPERTY, CONSIDERATION SHALL BE
GIVEN TO THE SUPERVISION AND/OR MONITORING OF CONDITIONS THAT MAY
CAUSE, OR MAY LEAD TO, A REAL OR POTENTIAL HAZARD ON ANY
INSTALLATION.
2) ALL SAFETY DEVICES SHALL BE LISTED FOR THE SERVICE INTENDED. SAFETY
DEVICES SHALL BE APPLIED AND INSTALLED WITH ESTABLISHED SAFE
PRACTICES AND MANUFACTURER'S INSTRUCTIONS.
3) FUEL-FIRED UNITS SHALL BE PROVIDED WITH ALL SAFETY DEVICES IN
ACCORDANCE WITH ESTABLISHED SAFE PRACTICES.
4) SAFETY CONSIDERATIONS SHALL EXTEND TO ALLIED EQUIPMENT, AND OTHER
PROXIMATE EQUIPMENT, TO AVOID ADDITIONAL CONTRIBUTORY HAZARDS.
ABATEMENT OF THIS HAZARD COULD INCLUDE THE INSTALLATION OF SAFETY
EQUIPMENT WITH AN AUTOMATIC CARBON MONOXIDE MONITORING SYSTEM, AN
AUDIBLE AND VISUAL ALARM DEVICE AND AUTOMATIC SHUT DOWN IN THE EVENT
OF POTENTIAL AUTOIGNITION OF COMBUSTIL GAS MIXTURE(S).

STIPULATED BY STANDARD HANDBOOK FOR MECHANICAL ENGINEERS, SEVENTH
EDITION, CHAPTER 9. ESSENTIAL ELEMENTS OF A WATER TREATMENT SYSTEM WOULD
INCLUDE:
1) REMOVAL OF SUSPENDED SOLIDS.
2) CHEMICAL TREATMENT FOR REMOVAL OF HARDNESS.
3) CATION EXCHANGE FOR REMOVAL OF HARDNESS.
4) DEMINERALIZATION FOR COMPLETE REMOVAL OF DISSOLVED SOLIDS.
