Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the hazard of falling crane loads. Among others, several feasible methods to correct this hazard is to comply with American National Standard Institute ANSI B30.15, Mobile Hydraulic Cranes: (1) Section 15-1.1.3 - A substantial and durable rating chart with clearly legible letters and figures was not provided and securely fixed to the crane cab in a location easily visible to the operator while seated at his controls; (2) Section 15-1.3.2(d) Telescoping boom cranes with less than 60 feet of extended boom was not provided with a two-blocking damage prevention device capable of preventing damage to the hoist rope or other machine components; (3) Section 15- 1.9.1(b) - A boom angle indicator readable from the operator station was not provided; (4) Section 15-1.9.1(c) - A boom length indicator readable from the operator station was not provided on telescoping booms.
