employer did not furnish employment and a place of employemnt which were
free from recognized hazards that were causing or likely to cause death
or serious physical harm to employees in that employees were exposed to
the hazard of unexpected or inadvertent activation of equipment:
a) On or about December 20, 1989 and at times prior thereto, millwright
performed maintenance and trouble-shooting on drive chain and
and sprocket of number one roll case without disconnecting and
locking out energy sources while headrig operator used the on and
off switch to determine whether maintenance was completed, resulting
in serious injuries to the millwright.
Among other methods, one feasible and acceptable abatement method to
correct this hazard is:
1) To develop specific procedures for maintenance operations in this
plant.
2) Develop an accountability system to ensure employees are following
lockout procedures.
3) Implement a step-wise enforcement program for use of lockout.
4) Periodically evaluate the program for effectiveness.
