replaced by cardboard, restricting vision of loader operator.
Among other methods, one feasible and acceptable a safe distance from
correct this hazard is:
1) Require employees to maintain a safe distance from loading and
skidding operations.
2) Repair rear cab window on Serco loader.
3) Implement formal written job training and accident prevention program
to include:
a) Orientation of new employees.
b) Monthly safety meetings.
c) Frequent visual observation of work practices.
