Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The
employer did not furnish employment and a place of employment which were
free from recognized hazards that were causing or likely to cause death
or serious physical harm to employees in that employees were exposed to
the hazard of crushing injuries from automotive lift collapse:
a) 120 South Main, Ketchum, Idaho-Weaver Corporation Auto Lift, Model
AFM-50-A, was not installed according to manufacturer's
specifications. Manufacturer's specifications requested the vertical
upright to be anchored level to the floor in good quality concrete
of at least six inches thick. Uprights were welded to two steel
plates (8'x4' x 1/2"); observed January 11, 1990.
Among other methods, one feasible and acceptable abatement method to
correct this hazard is:
1) Mount vertical upright base plates to concrete floor in accordance
with manufacturer's specifications (see Installation Section,
paragraphs 1 through 17, (pages 2 through 5) of the Owners
Installation and Service Manual).
2) If installation methods deviate from manufacturer's specifications,
written approval from the manufacturer must be obtained.

to ensure that all componenets are in safe operating conditions;
observed January 11, 1990.
b) 120 South Main, Ketchum, Idaho - Air-Oil Hydraulic Lift (manufacturer
and date of manufacturing unknown); the employer had not conducted
periodic inspections to ensure that all componenets are in safe
operating conditions; observed January 11, 1990.
Among other methods, one feasible and acceptable abatement methods to
correct this hazard is:
1) Conduct periodic inspections (at least annually) of all auto lifts
in accordance with manufacturer's specifications as required by
American National Standards Institute, Inc., B153.1-1974 (Safety
Requirements for The Construction, Care, and Use of Automotive Lifts)
paragraph 6.3.
2) Maintain and repair all noted deficiencies found during the
periodic inspection prior to returning to service.
3) Ensure that periodic inspections verify compliance with ANSI B.153.1-
-1974, paragraphs 3 (construction), 5 (installation), and 6
(Maintenance, Use, and Inspection).
