Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not furnish employment and a place of employment which were
free from recognized hazards that were causing or likely to cause death
or serious physical harm to employees in that employees were exposed to
falling hazard.
a) Type of wire rope, the number of wire rope utilized and the method of
attaching the wire rope to the elevator and counter weight did not
meet the requirements of ANSI.1-1971 in that:
1) Section 212, Rule 212.1 - Prohibits the use of standard wire rope,
wire rope utilized for elevator useage shall be classified as
"elevator rope".
2) Section 212, Rule 212.4 - Prohibits the use of one (1) wire rope
used in the conveyance system, the minimum useage of two (2) ropes
be utilized with elevator/counter weight systems.
3) Section 212, Rule 212.9a - Prohibits the use of "u" type clips used
for the attachment of wire rope to the elevator and to the counter
weight, attachment of roper to the elevator and counter weight shall
use a tapered babbitted rope socket.
