Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not furnish employment and a place of employment which were
free from recognized hazards that were causing or likely to cause death
or serious physical harm to employees in that employees were exposed to:
a) On or about May 24, 1989 and at times prior thereto, employees
trimming cant with an ax in close proximity to the feed chain to the
gang saw were exposed to the hazard of becoming entangled in the
moving chain, by working on and around the moving chain or by having
the chain inadvertently activated during the trimming process.
Among other methods, one feasible and acceptable abatement method to
correct this hazard is to develop, implement and enforce a lockout
policy such as that stipulated by ANSI Z244.1-1982, American National
Standard of Minimum Safety Requirements For Lockout/Tagout of Energy
Sources For Personnel Protection, Section 4 and 5. Essential elements
of the lockout/tagout procedure would include:
1) Establish a written lockout policy including procedural and training
requirements for all affected employees, both maintenance and
operational crews;
2) Provide lockout training for all affected employees, including both
maintenance and operational crews;
3) Before any maintenance, inspection, cleaning, adjusting or servicing
of equipment (electrical, mechanical, or other) that requires
entrance into or close contact with the machinery or equipment, the
main power disconnect switch or valve, or both, controlling its
source of power or flow of material, shall be locked out or blocked
off with a padlock, blank flange, or other effective means.
Residual energy in equipment having systems such as pneumatic,
hydraulic, compressed springs or other, shall have these systems
bled off, blocked or other effective means implemented to create a
zero energy state;
4) Provide each employee, working in areas requiring a lockout, with
individually assigned locks, keyed separately, to ensure removal only
by the employee(s) locking out the equipment;
5) Strickly enforce all provisions of the lockout policy;
6) Periodically evaluate the lockout policy for effectiveness.
