Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The
employer did not furnish employment and a place of employment which were
free from recognized hazards that were causing or likely to cause death
or serious physical harm to employees in that employees were exposed to
the hazard of being crushed:
Stafford Machinery Company, Yard Area - In accordance with
ASME/ANSI B30.5-1989, 5-3.1.3(1), the manufacturer's recommendations
were not followed nor was blocking present for the boom disassembly
procedure before the two (2) lower boom pins were removed from the
Bucyrus-Erie 30-B. Serious 4, super crawler crane (serial number
132006), on or about 11/01/90.
Among others, some feasible and acceptable abatement methods to correct
this hazard are: (1) follow the manufacturer's recommendations for
proper boom assembly and disassembly methods, an (2) post proper warning
labels at each boom connection point.
