Section 201 of the Executive Order 12196 of 1980:  The employer did not
furnish
to employees places and conditions of employment that were free from
recognized
hazards that were causing or likely to cause death or serious physical
harm to
employees in that:
a)Conditions similar to what had caused two previous injuries,
park management allowed employees to work down slope from
where rocks were being intentional dislodged.
Management did not require trail crew members to use
techniques which would greatly reduce the hazard of
being struck by boulders.  Among other methods, one
feasible and acceptable abatement method to correct this
hazard is to require employees to position themselves
horizontally, working from the top down.  When rocks and
boulders are being intentionally dislodged, other
workers must not be allowed to work down hill.

a)Park Management knowingly allowed trail crews to operate in
remote, difficult to access terrain, in known hazardous
operations without adequate radios or other communication
devices which would allow them to report injury situations and
summons help.  The area at Tapeats Creek is known to be an
area which standard park communictors are inadequate.
Among other methods, one feasible and acceptable abatement method to
correct this hazard is a satellite telephone, the installation of
radio repeaters or the purchase of a narrow band system.

a)Park Management does not have an adequate plan or method of
backup for emergency rescue and evacuation for employees
injured in remote locations.  Park Management assigns work
when helicopter rescue services are not possible because of
weather conditions.  One acceptable abatement method might be
to avoid high risk work when environmental conditions prohibit
or restrict helicopter rescue.
Among other methods, one feasible and acceptable abatement method to
correct this hazard is to prohibit rock/boulder clearing operations
when environmental conditions prohibit or restrict helicopter rescue
or ensure that an on-site employee has the capability and supplies
needed to stabilize the victim until emergency resuce servies
arrive.
