The employer did not furnish employment and a place of employment
which were free from recognized hazards that were causing or likely
to cause death or serious physical harm to employees in that
employees were exposed to:
(a)  Hazard of being struck by pieces of PVC pipe and/or
compressed air from broken PVC pipes installed in the
preparation area, warehouse area, and warehouse storage areas
designated ASTM D2241 and D1785 and used to carry compressed
air on or about August 12, 1993, and the piping is not
intended for that use.
Among other methods, one feasible and acceptable abatement method
to correct this hazard is to provide metal piping which is designed
for the purpose and service to which it is
