free from recognized hazards that were causing or likely to cause death
or serious physical harm to employees in that employees were exposed to
hazard of being struck by refuse collection truck in that a neutral
position interlock was not provided on vehicle equipped with an automatic
transmission which throttle is automatically advanced when compacting or
other auxiliary equipment is operated:
(a)  Worksite - Refuse Collection Truck No. 360.
Among other methods, one feasible and acceptable abatement
method to correct this hazard is to install a neutral position
interlock.
(b)  Hazard of being struck by refuse collection truck, in
that, prior to placing equipment into service, the employer
did not repair mechanical malfunction or breakdown that
affects the safe operation of the equipment:
1).  Worksite - Refuse Collection Truck No. 360, gear
range indicator lights on its Stone Bennett shift
control were defective.
Among other methods, one feasible and acceptable abatement
method to correct the hazard is to replace the defective
shift control.
(c)  Hazard of being struck by refuse collection truck, in
that, controls were not conspiciously labeled as to their
function.
1).  Refuse Collection Truck No. 360 - The toggle switch
for throttle advance and press gear located in center
of cab were not labeled.
Among other methods, one feasible and acceptable method to
correct the hazard is to label the controls.
