Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to a recognized hazard of electrical shock or burns:
(a)F/T Northern Jaeger:  The employer's lockout/tagout program did not
contain
lockout/tagout procedures for each individual piece of machinery or
equipment
which had more than one source of energy or the potential of stored
energy.
This
condition exposed employees to possible electrical shock or burns in the
event all
energy sources were not isolated during machinery and equipment repair.
Among other methods, one feasible and acceptable abatement method to
correct
this hazard would be to develop, document, and utilize energy control
procedures
for each individual machine or piece of equipment.
(b)F/T Northern Jaeger, galley:  Three 220-volt electrical panels did not
have a
minimum of three feet front working space and access to the panel was
blocked.
This condition exposed employees to possible electrical shock or burns in
the
event of contact with and/or repairs to machinery or equipment that cannot
be
deenergized
quickly due to blocked access to electrical panels.
Among other methods, one feasible and acceptable abatement method to
correct
this hazard would be to remove the supplies blocking access to the
electrical
panels.
(c)F/T Northern Jaeger, trawl deck, port side:  Employee(s) had occasion
to work
near and/or use a portable hand grinder powered by an extension cord.  The
ground prong was missing from the extension cord and the outer insulation
was
frayed adjacent to the female connection.  This condition exposed
employee(s) to
possible electrical shock or burns in the event of contact with bare
electrical
conductors or energized equipment.
Among other methods, one feasible and acceptable abatement method to
correct
this hazard would be to discard the extension cord.
(d)F/T Northern Jaeger, outside rain gear room:  Employee(s) had occasion
to work
near a hatch door with an electrical extension cord running through it to
a
110volt
submergible pump.  This condition exposed employees to possible
electrical
shock or burns in the event the hatch door was closed on the extension
cord and
the insulation abraded.
Among other methods, one feasible and acceptable abatement method to
correct
this hazard would be to remove the extension cord from metal pinch points.
