Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to a recognized hazard of electrical shock or burns: (a)M/V Yardarm Knot: In the Starboard Crane Cab is a duplex receptacle with reversed polarity when tested with Woodhead electrical tester. This condition exposed employees to possible electrical shock or burns. Among other methods, one feasible and acceptable abatement method to correct this hazard would be to remove and properly rewire the receptacle. (b)M/V Yardarm Knot: In the Engine room is a junction box with exposed electrical conductors. This condition exposed employees to possible electrical shock or burns when an event of an accidental contact. Among other methods, one feasible and acceptable method to correct this hazard would be to replace the missing cover. (c)M/V Yardarm Knot: In the old Laundry room is a duplex with reversed polarity when tested with Woodhead electrical tester. This condition exposed employees to possible electrical shock or burns. Among other methods, one feasible and acceptable abatement to correct this hazard would be to remove and properly rewire the receptacle. (d)M/V Yardarm Knot: At the Main deck fuel station was flexicord used for permanent wiring. This condition exposed employees to possible electrical shock or burns. Among other methods, one feasible and acceptable abatement method to correct this hazard is to install hard wiring at the fuel station.
