Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to a recognized hazard of electrical shock or burn:
(a)M/V Pribilof:  In the engine room, employees had occasion to work near
four
electrical outlet with reverse polarity when tested with Woodhead
electrical tester.
Among other methods, one feasible and acceptable abatement method to
correct
this hazard would be to remove and properly rewire the receptacles.
(b)M/V Pribilof:  In the engine room, a flexicord was used for electrical
wiring for
fluorescent lights.  This condition exposed employees to possible
electrical shock
or burn in the event of accidental contact with bare electrical conductors.
Among other methods, one feasible and acceptable abatement to correct this
hazard would be to remove the temporary wiring and install permanent
wiring to
the lights.
(c)M/V Pribilof:  In the engine room, the access stairway light was burned
out,
exposing employees to multiple injuries from tripping/slipping hazard.
Among other methods, one feasible and economical abatement method to
correct
this hazard would be to replace the burned out light bulbs.
(d)M/V Pribilof:  In the machine shop, there were four fluorescent lights
under 7
feet without proper covers or guards.  This condition exposed employees to
possible electric shock or burn in the event the light tubes were
accidentally
broken.
Among other methods, one feasible and acceptable abatement method to
correct
this hazard would be to install guards over the light tubes.
(e)M/V Pribilof:  In the refer room, the junction box cover was missing,
exposing
live wires to employees and the hazard to electrocution.
Among other methods, one feasible and economical method to correct this
hazard
to install a junction cover.
(f)M/V Pribilof:  In the Machine shop a 3" x 6" control junction box has a
missing
cover exposing employees to the hazard of electrical shock.
Among other methods, one feasible and acceptable method to abate the
hazard is
to install a control junction cover.
(g)M/V Pribilof:  In the weather deck behind the crane control box, is a
weatherproof receptacle without cover.  This condition exposed employees to
possible electrical shock or burn in the event the outlet is utilized and
energized
for other operations on deck.
Among other methods, one feasible and economical method is to install a
weatherproof receptacle cover.
(h)M/V Pribilof:  In the Generator room, employees had occasion to work
near an
electrical outlet with reverse polarity when tested with Woodhead
electrical tester,
exposing employees to hazards of electrical shock or burn.
Among other methods, one feasible and acceptable method to correct this
hazard
is to remove and properly rewire receptacles.
