Section 5(a)(1) of the Occupational Safety adn Health Act of 1970:  The
employer did not furnish employment and a place of employment which
were free from recognized hazards that were causing or likely to cause
death or serious physical harm to employees in that employees were
exposed to a recognized hazard of electrical shock, burns and or
electrocution:
(a)  M/V Western Sea:  The breaker box panel labeled GL 2, used
to provide power to the state room heaters, located in the galley,
was not equipped with a dead front or faceplate. This condition
exposed employee(s) to the hazard of contacting live 120/220 volt
circuits.
Among other methods, a feasible and acceptable abatement method to
correct this hazard is to install a dead front or faceplate to
prevent employees from contacting the live electrical parts.

Section 5(a)(1) of the Occupational Safety adn Health Act of 1970:  The
employer did not furnish employment and a place of employment which were
free from recognized hazards that were causing or likely to cause death
or serious physical harm to employees in that employees were exposed to
the recognized hazard of being struck by the descending or ascending
elevator, falls into the elevator shaft, crushing injuries,amputations
and/or death:
a) M/V Western Sea:  In the freezer, the elevator to the box storage
area was not equipped with a case on the platform to protect
employee(s) from falling off the elevator platform and to protect
them from the moving elevator chain and from shear points between
elevator floors.  These doors were not interlocked at the top of the
elevator to prevent them from opening when the elevator was not at
that level and to protect employee(s) from falling into the elevator
shaft. There was no guarding of the bottom of the elevator to
protect employee(s) from stepping under it while it was descending.
Among other methods, a feasible and acceptable abatement method to
correct this hazard is to not allow employee(s) to ride the elevator
and provide another access by cutting a hole in the freezer floor and
installing a fixed ladder which would be used to access the box storage
area. At the top of the elevator either install guardrailing to prevent
employee(s) from falling into the ladder way opening, or install a trap
door which could be opened and closed as needed to access the fixed
ladder. Interlock the doors on the freezer so they can not be opened
when the elevator is not a that level and provide guardrails at the base
of the elevator to prevent employee(s) from stepping under it while it
is descending.

employer did not furnish employment and a place of employment which
were free from recognized hazards that were causing or likely to cause
death or serious physical harm to employees in that employees were
exposed to electrocution,  electrical shock or burns, and cuts and
lacerations due to contact with exposed current carrying parts and
broken glass:
(a)  M/V Western Sea:  In the machine shop area, the incandescent
light at the top of the stairs, approximately 6 feet, 2 inches
above the floor was not equipped with a bulb guard.  This
condition exposed employee(s) to the hazard of electrocution,
shock or burns, and cuts and/or lacerations.
(b)  M/V Western Sea:  In the sewage room adjacent to the gray water
tank, the incandescent light located about 6 feet, 2 inches
above the floor was not equipped with a bulb guard.  This
condition exposed employee(s) to the hazard of electrocution,
electrical shock or burns, and cuts and/or lacerations.
(c)  M/V Western Sea:  In the aft steering room near the emergency
steering motor, the incandescent light, approximately 5 feet,
9 inches above the floor level, was not equipped with a bulb
guard.  This condition exposed employee(s) to the hazard of
electrocution, electrical shock or burns, and cuts and/or
lacerations.
(d)  M/V western Sea.  In the engine room, the incandescent lights,
6 feet, 6 inches and 5 feet, 10 inches above the floor were not
equipped with bulb guards.  This condition exposed employee(s)
to the hazard of electrocution, electrical shock or burns, and
cuts and/or lacerations.
(e)  M/V Western Sea:  In the lazzerett area, the florescent light
5 feet, 11 inches off the floor was not equipped with a bulb
guard.  This condition exposed employee(s) to the hazard of
electrical shocks or burns, and cuts and/or lacerations.
(f)  M/V Western Sea:  IN the compressor room, the florescent lights
located about 6 feet off the floor werenot equipped with a bulb
guard.  This condition exposed employees to the hazard of
electrical shock or burns, and cuts and/or lacerations.
(g)  M/V Western Sea:  In the tool room, the florescent light located
over the gasket bench about 5 feet, 11 inches off the floor was
not equipped with a bulb guard.  This condition exposed
employees to the hazard fo electrical shock or burns, cuts,
and/or lacerations.
Among other methods, one feasible and acceptable abatement
method to correct this hazard is to attach bulb guards to
lighting fixtures so that the bulbs can not be accidently
contacted or broken.
