Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not furnish employment and a place of employment which
were free from recognized hazards that were causing or likely to cause
death or serious physical harm to employees in that employees were
exposed to hazardous energy:
(a)  F/V Yukon Viking:  No written procedures had been developed
and implemented on the lockout/tagout program to inform
employees of the hazards involved, and the precautions to
take when working on or repairing processing, mechanical,
and electrical equipment in accordance with ANSI STD.Z244.1
and 29 CFR 1910.147.
Among other methods to abate this hazard, one feasible method
would be to implement a program using the samples provided
to you and train your employees.

were free from recognized hazards that were causing or likely to cause
death or serious physical harm to employees in that employees were
exposed to electrical hazards:
(a)  F/V Yukon Viking:  Various electrical wiring(s) in the
hydraulic tool and engine rooms were inadequately guarded
to prevent accidental contact with live parts in accordance
with 29 CFR 1910.303(g)(2) and National Electrical Code 70
article 230.
Among other methods, one feasible and acceptable method to
correct these hazards would be to conduct an evaluation and
examination of all electrical wiring on board the vessel for
practical safeguarding, and then make the necessary repairs
for prevention.

(a)  F/V Yukon Viking:  In the tool room, a Copeland refrigerator
compressor was not guarded to prevent employees from
accidental contact of live parts in accordance with
1910.303(g)(2) and National Electrical Code 70 article
445-6.
Among other methods, one feasible and acceptable method
to correct this hazard would be to replace the missing cover
with the original cover or replacement cover.
