Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not furnish employment and a place of employment which
were free from recognized hazards that were causing or likely to cause
death or serious physical harm to employees in that employees were
exposed to hazardous energy:
(a)  F/V Pacific Orion:  No written procedures had been developed
and implemented on the lockout/tagout program to inform
employees of the hazards involved and the precautions to take
when working on repairing processing, mechanical and electr-
ical equipment in accordance with 29 CFR 1910.147 and ANSI
Standard Z244.1.
Among other methods to abate this hazard, one feasible
method would be to implement a program using the samples
provided to you and train your employees.
