Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not furnish employment and a place of employment which
were free from recognized hazards that were causing or likely to cause
death or serious physical harm to employees in that employees were
exposed to:
(a)  M/V Palisades:  No written procedures had been developed and
implemented on the lockout/tagout program to inform employees
of the hazards involved, and the precautions to take when
working on or repairing processing, mechanical, and electr-
ical equipment in accordance with ANSI Standards Z244.1 and
29 CFR 1910.147.
Among other methods to abate this hazard, one feasible method
would be to implement a program and train employees using the
sample provided.
Yard Arm Knott Inc. was previously cited for a violation of
this Occupational Safety and Health Standard or its equival-
ent standard 29 CFR 1910.147(c)(1) which was contained in
OSHA Inspection Number 107107476, Citation Number 1, Item
Number 4, issued on 09/27/90.
