Section 5(a)(1) of the Occupational Safety and Health Act of 1970:
The employer did not furnish employment and a place of employment which
were free from recognized hazards that were causing or likely to cause
death or serious physical harm to employees in that employees were
exposed to:
(a) One employee was observed leaving the scaffold near
the port screw of the vessel "Fair Princess" on dry
dock No. 4 and was exposed to a fall of approximately
15 feet.  The employee crawled over the guardrail and
climbed down the corner support post of the scaffold.
(b) Employees using the staging located throughout the
vessel "Fair Princess" did not have a means of access
to the platform but had to crawl over or through the
guardrails exposing them to the hazard of falling.
(c) Employees accessing the stack from the weather deck
on the "Fair Princess" did not have a means to access
the ladder provided but instead climbed over the
guardrails exposing them to the hazard of falling.
Among other methods, one feasible and acceptable abatement
method to correct these hazards is to install access ladders
so that employees do not have to climb over guardrails.
