The alleged violations below have been grouped because they involve similar or related hazards that may increase the potential for injury resulting from an accident. Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to: (a) On or about October 28, 1992, and at times prior to, employees installing insulation and plywood panels to sides of the Circle J residential home were exposed to fall hazards exceeding 10 feet from a personnel platform mounted to a rough terrain forklift truck, or having the entire platform displaced from the tines, resulting in a fall and crush hazard. Among other methods, one feasible and acceptable abatement method to correct this hazard would include: 1. Secure the personnel platform to the forklift tines or carriage to prevent displacement. 2. Install standard guardrails around the perimeter of the work platform (42-inch high toprail, 21-inch high midrail, and 4-inch toeboards). 3. Secure employees on the platform with safety belts and lanyards to restrain them within the platform in the event the platform is tilted or suddenly moved. 4. Maintain a trained operator in a position to control the forklift truck at all times when the platform is occupied by personnel. 5. Place the truck in neutral and apply the parking brakes and chock the wheels.
