Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not furnish employment and a place of employment which
were free from recognized hazards that were causing or likely to cause
death or serious physical harm to employees in that employees were
exposed to:
(a)  On or about August 27, 1992, and at times prior to,
employees working at Coca Cola Bottling Company of Central South
Dakota, in Mitchell, South Dakota, were exposed to the hazard
of having body parts crushed due to contact with moving elevator
component parts or falling into the hoistway when the elevator
car was absent from floor levels.
The following conditions constitute violations of ANSI A17.1
elevator standards:
1.  The hoistway gate on the second floor could be opened while
the elevator car was absent due to lack of an interlock.
2.  The hoistway opening on the main floor was not provided with
interlocking door or gate.
3.  The hoistway area at the basement level was not enclosed
and provided with interlocked door or gate.
4.  The elevator car was not provided with interlocking car
gates.
5.  The elevator car was not provided with a weight capacity
notification.
Among other methods, feasible and acceptable methods of abatement
to correct the hazards would include:
1.  Equip the second floor hoistway gate with an interlock that
would preclude it being opened while the elevator car is absent.
2.  Install hoistway door or gate on the main floor and equip
it with an interlock.
3.  Enclose the hoistway area at the basement level and provide
hoistway door or gate with an interlock.
4.  Provide gates on the elevator car and equip them with
interlocks.
5.  Determine the elevators load capacity rating and post a load
capacity notice in the elevator car.
