employer did not
furnish employment and a place of employment which were free from
recognized hazards
that were causing or likely to cause death or serious physical harm to
employees in that
employees were exposed to:
(a) Employees were exposed to a potential fall hazard while operating the
manually
operated manlifts which were not provided with guardrails on all sides of
the manlift
car in the main elevator and the manlift car in the corn elevator. Among
other
methods, one feasible and acceptable method to correct this hazard would
be to install
a complete car enclosure.
(b) Employees were exposed to a potential crushing hazard while working
near the
base of the manlifts in case of a manlift failure due to the lack of
guardrails around
the base of the manually operated manlift in the main elevator and the
manlift
in the
corn elevator. Among other methods, one feasible and acceptable method of
abatement to correct this hazard would be to install guardrails with an
entry gate
around the base of each manlift.ls
