General-duty citation text
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to: (a) Employees were exposed to falling loads and/or falling hoist parts in that the jobmade jib hoist was not kept in good repair with the following deficiencies noted: (1) The rated load was not permanently marked on the hoist, (2) Load testing was not conducted with a written report to confirm the hoist, (3) There was no load ratings test conducted on the beam, (4) The bicycle type chain was broken, (5) The electric motor cables were wrapped around equipment because of their length, (6) The motor controls were not marked; (b) Preparation area, East side; The 5 ton capacity pendant controlled grinder hoist was not kept in good repair in that the following deficiencies were noted: (1) Load testing was not conducted with a written report to confirm the hoist load rating, (2) The wire rope was worn, (3) The 7/8 inch sheave was worn and the hook latch broken; (c) Preparation area, East side; 5 ton capacity pendant controlled grinder hoist; Running ropes and hoist parts were not inspected frequently or periodically nor were written reports that repairs were performed and parts replaced. Among other methods, one feasible and acceptable abatement method to correct this hazard is to develop a preventive maintenance program which incorporates all recommendations outlined in the manufacturer's manuals with written, signed and dated reports and also in accordance with ANSI B30.16 and ANSI B30.11 in order to ensure a safe and proper functioning hoist.oning
