Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The %% employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to: a) Alignment rods (tie rods) and/or pipe anchors were not adequately designed to withstand the pressure thrust forces created inside of a 36 inch diameter expansion joint when pressurized to approximately 65 psig contributing to it's failure. This condition occurred on or about February 13, 1992 in the heavy assembly and testing department.

The alleged violations below have been grouped because they involve %% similar or related hazards that may increase the potential for injury resulting from an accident. Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to: a) A 36 inch diameter expansion joint rate at 50 psig was used in a test piping system where the intent was to pressurize the system to roughly 100 psig. This condition occurred on or about February 13, 1992 in the heavy assembly and testing department.

Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The %% employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to: a) A program (e.g. job safety analysis) was not developed and implemented dealing with the utilization of expansion joints as found in the heavy assembly and testing department on or about February 13, 1992. Such a program must follow recommendations made by the manufacturer and the Expansion Joint Manufacturers Association and should include the following at a minimum: 1. Proper selection of expansion joints including recognition and interpretation of the manufacturer's part number. 2. Proper installation of expansion joints including the design of pipe anchors and/or tie rods where appropriate. 3. Maintenance and inspection procedures including the tracking of cycle life where applicable. 4. Proper storage and handling of expansion joints.
