Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The %% employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to: (a) Inadequate procedures for confined space entry into the Keystone Boilers in the Boiler House. 1. There was no written confined space entry program. 2. Employees and supervisors were not trained in safe confined space entry program. 3. No program developed to include a permit system, with management "sign off" (authorization) and hazard control check list. 4. No program developed to include provisions for testing for oxygen and combustible gas/vapor levels or monitoring the air for toxic materials. 5. No program developed to include the posting of warning labels and safety instructional signs in the area of confined space entry work. 6. No program developed to include specific training of the stand by worker on his/her function while confined space entry work is being performed or during an emergency. 7. No program developed to ensure that ventilation is adequate during confined space entry work. 8. No program developed to equip employees conducting confined space entry work with a lifeline and harness. 9. No program developed for a self contained breathing apparatus to be immediately available for confined space entry work and emergencies when conditions warrant the use of such a breathing apparatus. Among other methods one feasible and acceptable method to correct this hazard is to develop and implement a confined space entry program.
