Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The
employer did not furnish employment and a place of employment which were
free from recognized hazards that were causing or likely to cause death
or serious physical harm to employees in that:
A) Employees were exposed to being struck by building components
due to structure collapse at the Airside Building at the Midfield
Terminal Project of the Greater Pittsburgh International Sirport
in that:
1) The roof beam RB-35 at column line B20 failed in flexural
due to the inadequate development length of its bottom bars
marked #701.
2) The flexural reinforcement of two #7 bars of beam RB-35
to support the construction loads during erection was marginal
as per ACI 31B-83.
3) The lap splice length of #7 bars marked #701 and #702
did not meet the aci design criteria.
4) Four #8 bars for the longitudinal bottom reinforcement of
beam RB-35 were not properly spaced by the precast manufacturer.
5) Field examination of the embedded reinforcing steel had
indicated a series of inconsistencies in placement of bars.
