Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not furnish employment and a place of employment which
were free from recognized hazards that were causing or likely to cause
death or serious physical harm to employees in that employees were
exposed to:

recognition and avoidance of unsafe conditions and the regulations
applicable to his work environment to control or eliminate the hazards
or exposure, associated with the operator placing his hands or fingers,
into the point of operation while the press brake was under power to
cycle.
Among other methods, a feasible and acceptable abatement method to
correct this hazards is to install barrier guards; pull-out devices;
holdout or restraint devices; two hand control devices; use hand
feeding tools or automatically feed.
