Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing of likely to cause death or serious physical harm to employees in that employees were exposed to the hazard of falls from 60' high catwalks mounted at and above the front and back blue silos, in that the expanded metal floor crating was loose and unattached, on or about January 5, 1993. a. Back Blue Silo #1 - Above Silo b. Back Blue Silo #3 - Above Silo c. Back Blue Silo #3 & 4 - Between Silos d. Back Blue Silo #4 - Above Silo e. Back Blue Silo #5 - Above Silo f. Front Blue Silo #1 - Above Silo g. Front Blue Silo #2 - Above Silo h. Front Blue Silos #1 & 2 - Between Silos Among other methods, one feasible and acceptable abatement method to correct this hazard is to replace broken or damaged or bent floor grating and weld the new grating into position.
